Category: Aminoglycoside antibiotics (exert bactericidal effect by inhibiting the 30S subunit of bacterial ribosomes).

Mechanism of action

It is the same as other aminoglycoside drugs. By binding to the 30S subunit of the bacterial ribosome, it interferes with mRNA translation, leading to the synthesis of incorrect proteins. It belongs to concentration-dependent bactericidal drugs.

Antibacterial Spectrum

·Gram-negative Bacteria:Highly sensitive to Enterobacteriaceae (such as Escherichia coli, Klebsiella, Proteus, Salmonella, Shigella), and also has good activity against Pseudomonas aeruginosa (slightly   weaker than gentamicin).

·Still effective against some β-lactamase-producing strains (such as ESBL-producing Escherichia coli), but has poor effect on AmpC enzyme or carbapenemase-producing drug-resistant bacteria.

·Gram-positive Bacteria:Effective against Staphylococcus aureus (including methicillin-sensitive strain MSSA), but has weak effects on Streptococcus and Enterococcus, and is ineffective against anaerobes.

·Resistance:Not easily inactivated by common aminoglycoside-inactivating enzymes (such as AAC (3)-II, APH (3’)-III), so it is still effective against some gentamicin- and tobramycin-resistant strains.

Severe infections caused by susceptible bacteria:

·Respiratory tract infections: such as pneumonia, bronchitis (especially hospital-acquired pneumonia caused by Gram-negative bacteria).

·Urinary tract infections: pyelonephritis, cystitis (should be combined with β-lactams for complicated infections)

·Abdominal infections: Peritonitis, cholangitis (usually used in combination with anti-anaerobic drugs such as metronidazole).

·Skin and soft tissue infections: Abscesses, wound infections.

·Sepsis: Needs to be used in combination with other antibiotics.
