Acetylspiramycin is a macrolide antibiotic. It exerts antibacterial effects by inhibiting bacterial protein synthesis and is an acetylated derivative of spiramycin. Its antibacterial spectrum is similar to that of erythromycin, but its antibacterial activity is slightly weaker. It has the characteristics of good oral absorption, wide tissue distribution, and low toxicity.

Physical and Chemical Properties

Appearance: The pure product is a white or off-white powder, odorless, and bitter in taste.

Solubility: Slightly soluble in water, easily soluble in organic solvents such as methanol, ethanol, and acetone (acetylation modification enhances liposolubility).

Stability: Stable in a dry environment, and should be stored in a sealed container to prevent moisture absorption.

Antibacterial Spectrum and Mechanism of Action

1. Antibacterial Spectrum

Gram-positive bacteria: Staphylococcus aureus, Streptococcus, Streptococcus pneumoniae, etc. (still have certain activity against drug-resistant bacteria).

Some Gram-negative bacteria: Haemophilus influenzae, Moraxella catarrhalis.

Atypical pathogens: Mycoplasma, Chlamydia, Legionella, Toxoplasma, etc.

Note: There is cross-resistance with macrolide drugs such as erythromycin.

2. Mechanism of action

By binding to the 50S subunit of the bacterial ribosome, it inhibits protein synthesis, thus exerting bacteriostatic effect (it can be bactericidal at high concentrations).

3.Usage and Dosage

Adults: 0.2 - 0.3g (200,000 - 300,000 units) per dose, 4 times a day, taken after meals.

Children: 20 - 30mg/kg per day, taken in 3 - 4 divided doses.

Course of Treatment: Usually 5 - 14 days, which needs to be adjusted according to the type of infection.
