Structural Features: It contains a quaternary ammonium group, is strongly alkaline, and is easily soluble in water. Its hydrochloride salt (Berberine Tablets) is a commonly used clinical dosage form.

Pharmacological Actions and Mechanisms

Antibacterial Action

Mechanism:

Inhibits bacterial DNA topoisomerase II (gyrase), interfering with DNA replication.

Disrupts the permeability of the bacterial cell membrane, causing the leakage of intracellular components.

Antibacterial Spectrum:

It has inhibitory effects on both Gram-positive bacteria (such as Staphylococcus aureus, Streptococcus) and Gram-negative bacteria (such as Escherichia coli, Shigella dysenteriae), and is particularly effective against intestinal pathogenic bacteria (such as Shigella dysenteriae).

Limitations: Bacteria are prone to developing resistance to it, and caution is required when using it alone for severe infections

Clinical Applications

Infectious Diseases

Intestinal Infections:

It is applicable to mild to moderate bacillary dysentery and gastroenteritis (caused by Shigella, Escherichia coli, etc.), and is often used as a first-line drug.

Usage: Oral administration, 100 - 300 mg each time, 3 times a day, with a treatment course of 3 - 7 days.

Adjuvant Treatment for Diabetes: It is applicable to patients with type 2 diabetes (fasting blood glucose < 11.1 mmol/L), can reduce blood glucose by about 10% - 15%, and is often used in combination with metformin.

Usage: 300 - 500 mg each time, 3 times a day, and blood glucose needs to be monitored.
